Starting your treatment? Then you need a referral.
To start your rehabilitation at AM Medical Rehabilitation you need a valid referral from your general practitioner, medical specialist or other doctor, such as a sports physician or company doctor. They will send us a (digital) referral letter – you don't have to do anything yourself.
Referral Letter
For proper administrative processing by your health insurer, a referral letter is necessary for your first appointment. This letter should be addressed to AM Medical Rehabilitation and must include:
- Reason for referral
- Signature of referrer
- Date of referral
In addition, identification is mandatory. Please bring both your health insurance card and a valid passport or identity card to your first appointment.

Reimbursement from basic insurance
Medical Specialist Rehabilitation Care (MSRC) from AM Medical Rehabilitation is covered by the basic package of your health insurance.
Healthcare costs leniency scheme
AM Medical Rehabilitation believes it is important to provide people with more complex care needs quick access to effective care, without long waiting times. That is why AM Medical Rehabilitation has deliberately chosen not to conclude contracts with all health insurers.
As a result, your health insurer may not fully reimburse all costs, depending on the type of policy you have (such as an in-kind, combination, or internet policy). In that case, only a portion of the healthcare costs will be reimbursed. AM Medical Rehabilitation will cover the non-reimbursed portion of the costs, with the exception of your compulsory deductible and any voluntary deductible.
Deductible
Everyone in the Netherlands has a mandatory deductible of €385 per year (in 2025). If there is any outstanding deductible at the time of reimbursement, this amount will be deducted from the reimbursement by the health insurer. This applies to all healthcare providers.
Voluntary deductible
If you have chosen a voluntary deductible in addition to the mandatory deductible, you may have to pay extra costs. The costs for your treatment will then be set off against the remaining part of your mandatory and voluntary deductible. You can always ask your health insurer how much deductible is still outstanding and which reimbursements apply to your policy.

Our claims are processed through our WTZa institution, Stichting AM Medi Reva. The AGB code for this foundation is 22221063.
AM Medi Reva Foundation
AM Medi Reva Foundation is an accredited healthcare institution for medical specialist care. The institution is approved and registered in the General Data Management Healthcare Providers (AGB) register. Our AGB code is 22221063. This unique code identifies us to all health insurers in the Netherlands as an accredited MSRZ healthcare provider. The rates for our treatments (DBC) are set nationally by the Dutch Healthcare Authority (NZA).
